Description

As part of the expanded HHVBP model, organizations will face penalties if patients require hospitalization or ER care within the first 60 days of home health use. These soon-to-be-enforced penalty-based markers may feel inordinately daunting considering industry turnover rates at 64%. However, by knitting together the right partners, agencies can add critical tools to their care delivery toolbox. An agency specializing in the delivery of high acuity episodic healthcare in the home, and a large home health and hospice provider, have partnered with impressive success and will share with you solid strategies that address the complexities of the coming regulatory shifts.
